Student Access to Digital Technology

Parent permission is required for students to access digital technology as part of their curriculum.

This form is applicable for all enrolled students from Kindergarten to Year 6.  It needs to be completed and signed annually.

General Consent

Parents complete this form at the time of enrolment.  This covers the following topics: viewing consent / local excursion/incursion / headlice / student photo publishing / online services acceptable use agreement.

Student Healthcare Summary

Parents complete this form at the time of enrolment, and it is important that the school has the most up to date information about your child’s healthcare needs.  When changes occur, parents are required to inform the school and complete a new healthcare form.  This can be for temporary medications or changes in healthcare needs.

Starting school can be an exciting time for you and your child.  At Joondalup Education Support Centre we offer small classroom sizes with a high staff ratio from Pre-Kindergarten to Year 6.
